Responsibilities:
Ensures that employees receive compliance training and education necessary to perform their job responsibilities in accordance with Avamere Code of Business Conduct.
Oversees the process for competency evaluation of new clinical department personnel; recommends progression to permanent employment or extension of orientation/probationary period for new employees.
Plans, organizes, and implements ongoing education and training programs for licensed nurses, nursing assistants, unit clerks, and other ancillary personnel to promote the knowledge and skills necessary for the provision of quality care consistent with services provided by the center.
In coordination with the Director of Nursing and the Administrator, periodically reviews the center Clinical Admission Grid and service lines to identify applicable advanced/as needed competencies required for clinical personnel.
Plan, develop, organize, implement, evaluate, coordinate, and direct our infection control program in accordance with current rules, regulations, and guidelines that govern such requirements in nursing care facilities.
Conducts or coordinates certification or training programs for Nursing Assistants to prepare them for state certification/competency exam as applicable.
Conducts needs assessment through review of survey history, individual performance and competency evaluation, center performance improvement trends, input from center department managers, and input from individual staff members, to identify educational needs to staff.
Provides and/or assists employees to identify available resources to achieve continuing education requirements if applicable.
Ensures the educational curriculum in the center meets or exceeds state, federal and accreditation requirements.
Develops and maintains monthly and annual education calendars.
Ensures educational opportunities are effectively communicated to center personnel.
Provides clinical and classroom hands-on instruction, and on the job coaching to licensed and un licensed personnel.
Coordinates and/or conducts CPR training for licensed personnel and applicable non-licensed personnel.
Working with the Director of Nursing Services (DNS), ensures clinical competency of nursing staff through observed skills evaluation, and ensures completion of annual competency assessments for clinical personnel.
Utilizes and disseminates current nursing literature and educational materials. Ensures staff awareness of current policies and procedures and professional standards of practice.
Ensures staff education and skills validation regarding patient care procedures and equipment.
Ensures that documentation for all training and education is completed and maintained in accordance with state, federal and Avamere Compliance program requirements.
Maintains organized record of continuing education for each employee.
Recommends staff development program and resource needs to administration for budgets and contains expenditures to current budge.
Develops and facilitates and orientation program for all facility personnel. Orientation programs to include general orientation for new and rehired facility personnel and clinical orientation for clinical employees in accordance with Avamere policy as well as state and federal requirements and other job specific/roles based training as needed.
Reviews new hire paperwork to ensure appropriate documentation is present prior to employee working.
Plans, organizes and implements concise orientations for contract personnel.
Coordinates students' clinical experiences within the facility; provides orientation for visiting faculty and students.
Ensure that the facility is in compliance with current CDC, OSHA, and local regulations concerning infection control or standard/universal precautions.
Develop, maintain, and periodically update infection control precautions and aseptic technique that successfully correct problem areas.
Monitor infection control practices and procedures to ensure that all personnel are implementing our standard operating procedures for tasks involving exposure to blood/body fluids.
Serve on, participate in, and attend Infection Control Committee meetings.
Develop and implement a practical system of detecting, evaluating, recording, and reporting infections that originate with facility personnel.
Participate in the interviewing and selection of residents for admission to the facility.
Assist the Director of Nursing Services in preparing the budget for the department in relation to its infection control program and activities.
Develop and participate in the planning, conducting, and scheduling of timely in-service training classes and educational programs that provide instructions on "how to do the job" for all facility personnel.
Assist in the development, implementation, and maintenance of a written plan of care (preliminary and comprehensive) for each resident in isolation or on infection control precautions that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
Identify, evaluate, and classify routine and job-related functions to ensure that tasks in which there is potential exposure to blood/body fluids are properly identified and recorded.
Qualifications:
Must possess a nursing degree from an accredited college or university.
Must possess a current, unencumbered, active license to practice as a RN in this state.
Must have, as a minimum, two (2) years clinical experience in a hospital, nursing care facility, or other related healthcare facility.
Must have, as a minimum, six (6) months training experience in rehabilitative and restorative nursing practices.
Must possess a working knowledge of epidemiology, microbiology, infectious diseases, and aseptic technique to include Standard Precautions.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities and infection control practices, to include standard/universal precautions.
Must maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
Must chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident and resident's response to care.
Experience with Electronic Medical Records and computer documentation systems.
Effective communication, organization and prioritization skills.
Customer service skills/experience required.
Knowledge of reimbursement programs, Medicare and Medicaid preferred.
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
Must be able to read, write and speak English fluently.
Three (3) years of nursing experience, of which one (1) year was in a long term care environment.
One (1) year experience as an instructor preferred.
One (1) year acute care experience preferred.
Must speak, read, and write English fluently
Must have an active CPR/BLS certification
Education/Certifications:
Graduate of an accredited school of nursing, RN required, BSN degree preferred
Active RN license in the state of Oregon
Valid CPR teaching certificate preferred
IV Instructor Certification preferred
Certified Infection Control Nurse preferred
ACLS certification preferred
Current with state Continuing Education (CE) requirements
