Family Partner Coordinator (Hybrid, Charlotte, North Carolina Based) (Personal Services)
The Family Partner Coordinator works to support local care management and care coordination efforts through a combination of activities. They work closely with System of Care Coordinators and adhere to the System of Care values and principles to help families, and their children succeed at home, in school, and in the community. Further they foster relationships and partnerships to aid in the identification and referral of individuals that meet special needs or high-risk criteria. Family Partner Coordinators are well-versed in System of Care principles and are individuals credentialed as Family Peer Support professionals.
This position will allow the successful candidate to work a primarily remote schedule. One day a week working onsite is required.

Minimum Requirements
Education & Experience
Bachelor's degree and at least three (3) years of progressive experience in the field of Health Care or Human Services.
Special Requirements
Must be a parent or caregiver of a child who has/have received services from the one or more of the System of Care community's child serving agencies.
Must have National Certification in Family Peer Support within one year of hire date.
Must obtain CFT Train the Trainer Certification within six
months of hire (dependent upon availability of some external training)

Salary Range
$25 - $31.88/Hourly
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
